Amend A Maryland Birth Certificate After Adoption

You'll have to do lots of paperwork to amend a Maryland birth certificate.


Amending a birth certificate is different than submitting a name change or an adoption form. Once a birth certificate is changed, no record of the previous information should be considered valid. In Maryland, birth certificates are held by the Office of Vital Statistics. To amend a birth certificate, you'll have to have a court order. If you're applying for your adopted child, you have the right to make the changes as her legal parent.


Instructions


1. Obtain a copy of your court order requesting a name change, sex change or parent change on the birth certificate. Even if you're changing it for a child, you'll still have to file a Petition for a Name Change in your local court.


2. Bring a copy of the adoption paperwork that certifies you as the adoptive parent. If there are two adoptive parents, you'll have to bring the other parent with you as well.


3. Visit the Office of Vital Statistics at:

4. Present your information, along with your identification. You'll need to know the name of the child on the birth certificate, where she was born and the names of the biological parents.


5. Pay the required fee with cash, check or a credit card. You will receive a new copy of the amended birth certificate in the mail.
