Get A Birth Certificate In Connecticut

Apply for a Connecticut birth certificate.


The Connecticut Department of Public Health recommends applying for birth certificate copies to the vital records office of the registrant's town of birth. For those residing outside of the state, application materials are available through the Department of Public Health's website. Family members and others associated with the registrant are also eligible to receive a birth certificate copy with certain restrictions.


Instructions


1. Make sure you are eligible to apply for a birth certificate if you are not the registrant. You must be an immediate relative, a state or federal agency employee, a legal representative of the registrant, an elected official of the registrant's town of birth, the director of health of the registrant's town of birth or a member of an authorized incorporated genealogical society to be eligible. You must provide evidence of your eligibility to apply for the birth certificate, such as your birth certificate if you are the registrant's child, or a written release from the registrant if you are from a state or federal agency.


2. Visit the Connecticut Department of Public Health's Vital Records website at http://www.ct.gov/dph/cwp/view.asp?a=3132&q=390652. Click on the "Application to Request a Birth Certificate" link. Click on "Application for a Birth Certificate from City/Town" to download and fill out a request form. If you are unable to print a copy of this form, contact the city or town of birth to ask about hard copies.


3. Bring your application, authorization (if applicable), a photocopy of a government-issued photo ID and the fee of $20 (as of 2010) per birth certificate to your or the registrant's town of birth's vital records office. You may also apply by mail to the town's vital records office. Enclose your application, a photocopy of your ID and a money order for $20 per copy made payable to the town of birth.
